Hi! Thanks for entering the "Level Up at the Library" Teen Video Contest.
Before we can use your awesome video, we need your permission.

By signing below, you (and your parent/guardian if you're under 18) agree to the following:

I made this video myself and it’s my original work.

I give Timberland Regional Library permission to use my video (and my name, photo, and voice if they appear in the video) in any format, medium, or platform for promotional or informational purposes— i.e. website, YouTube, social media, ads, and other library promotions.

I understand I won't get paid for this, but I might win a prize if I’m a contest winner!

Timberland Regional Library can edit, share, and reuse my video however they need.

If I’m under 18, my parent/guardian says it’s okay too.
